Medications
Over the Counter/Non-prescription Medications:
HDSB staff are not permitted to store nor administer non-prescription medication. If your child requires over-the-counter medication at school (e.g. Tylenol), you will need to attend the school to administer it to your child yourself, or, alternatively, you can obtain a prescription from the healthcare provider and have it dispensed by the pharmacist with a prescription label.
Prescribed Medications Self-Administered by Students:
If your child can take their prescription medication independently (without adult support), you can complete the Authorization for Self-Administration of Prescribed Medication form. This form does NOT require the signature of a Healthcare Provider. We will store your child’s medication in the office, but your child will administer the medication independently.
Prescribed Medications Administered by School Staff:
If your child requires the support of school staff to take their medication (e.g. to measure the dosage), you must complete the Request for School Personnel to Administer Prescribed Medication form. This form DOES require the signature of a Healthcare Provider, so you will want to bring this with you to the appointment.
Prevalent Medical Conditions
All students with a Prevalent Medical Condition such as Anaphylaxis, Asthma, Diabetes, Epilepsy/Seizure Disorder etc. are required to have a Medical Plan of Care co-created with the School Team. If your student has a Medical Condition, please reach out to our Vice Principal, Jeff Burrows at burrowsj@hdsb.ca.
Anaphylaxis
All students with anaphylaxis are required to carry their epi-pen with them at all times. We recommend the use of a fanny pack or cross-body bag for this purpose. A second epi-pen must be stored in the school office in case of emergency. A Request for School Personnel to Administer Prescribed Medication form is required.
Asthma
All students with asthma are required to keep their inhaler with them. We recommend the use of their backpack or a fanny pack for this purpose. A second inhaler must be stored in the school office in case of emergency. A Request for School Personnel to Administer Prescribed Medication form or Authorization for Self-Administration of Prescribed Medication form is required.
Concussions
Suspected Concussions
If your child sustains a jarring blow to the head, staff will complete an Appendix A - Suspected Concussion Form.
If your child has no signs or symptoms of a concussion they will need to be monitored at home and school for a period of 24 hours.
If you child has any signs or symptoms of a concussions you will need to pick them up from school and to consult with a medical practitioner. They will be required to rest at home for a minimum of 24 hours and to be monitored for concussion symptoms.
If your child has any RED FLAG symptoms, 911 will be called.
Diagnosed Concussions
If your child is diagnosed with a concussion, you will be provided with Appendix B - Home and School Concussion Management Plan Part A and Appendix B - Home and School Concussion Management Plan Part B and will need to complete a gradual return to school. Please note that your child will require Medical Clearance signed by a doctor or nurse at the end of Stage 4 in order to return to Physical Education.
